Document requirements:

• Your passport (including arrival/ departure card)

• Photocopies of your Passport (front page, Departure car Entry Stamp)

• Thai Visa Extension Form

• 1 self-portrait picture 4X6 C.M. (in 2 inches size)

• Full details of your address in Thailand
